How much will your students learn during the Walk On! Challenge? That’s what we want to know.
In your shipment of Walk On! Challenge tool kits, you should have received two sets of surveys: one for the students to take before they begin the Walk On! Challenge and one for them to take after they complete the Challenge. Please submit your pre-Challenge survey information below by February 10, 2012.
These surveys help Blue Cross Blue Shield of Arizona determine the students’ knowledge level about fitness, nutrition and healthy habits before and after the Walk On! program. This is also valuable information in shaping the future of this and other health focused curriculum.
Both the pre- and post-Challenge surveys MUST be completed and submitted electronically to be considered for the $5,000 grants from Blue Cross Blue Shield of Arizona. The surveys are anonymous and no students’ names are collected. Surveys are provided only to teachers who officially registered for the program.
Students should complete the pre-event survey before the February 1 kickoff to the Walk On! Challenge and teachers can submit results between January 16, 2012 and February 10, 2012.
To submit results, please have your students score their surveys based on the answer sheet provided in the tool kit, tally results and return them to you. You can then submit below.
Please report your results no later than Friday, February 10, 2012.
